You want to stay living in your own home, but you need support to feel safe and comfortable

A Home Care Package is money from the Government, aimed at supporting older people with specific needs to stay safe, comfortable and well at home. You can use this allowance alongside the support you get from family and friends.

You can choose annecto to help coordinate your Home Care Package and connect you to the people so you can feel safe and comfortable at home.

There are four levels of Home Care Packages:

Level 1 (basic) support may include cleaning, gardening or help with meal preparation.

Level 2 (low-level) may include the same as level 1 but with a nurse to visit and assist with medication management or wound care, twice a week.

Level 3 (intermediate) may include middle-level health care such as catheter or continence management, and mobility equipment such as walking frames, bed rails or on call support from medical staff.

Level 4 (high care) includes high care needs such as docter/nurse on call 24 hours, assistance with feeding, enteral feeding formula and equipment or allied referrals.

How can I arrange for annecto to be my organisation of choice?

One of our experienced Case Managers can provide free advice along the way.

Usually, your doctor will refer you to myagedcare.gov.au or you can call for advice on 1800 200 422.

The assessment team will visit you at your home, determine the level of care you require and send you a copy of their assessment. During the meeting, make sure you let the assessor know you would like to use annecto.

Call your local annecto office and book an appointment with a Case Manager.

An annecto Case Manager will meet with you at your home to discuss your needs and to develop a plan.

You approve your plan and agree to the fees before the plan begins.

Dementia Supplement

You may be eligible for a Dementia supplement. If a person is diagnosed with dementia, they can be eligible for a Dementia supplement that provides an extra 10 per cent in government funding on top of the relevant Home Care Package funding.

What will it cost?

As part of the planning process, an individualised budget for your home care package is prepared consisting of:

Package income

Government funding amount
Additional supplements eg. Demential supplement
Any contributions you would like to make.

Package expenditure

Administration cost
Service and supports based on what you have requested and the annecto fee schedule
Contingency fund (optional).

The costs will be discussed in detail at your first meeting.
