The most important ideal is for you to live well at home, connected to your family and friends, and being active in the community.

annecto offers guidance and resources for you to participate in your community, and to take responsibility for shaping your own life. This involves building your confidence, social engagement and ability to access information and learning in order to develop your skills, confidence and capabilities. Then we link you to networks and services in the community that will help you to be more self-reliant in finding the supports needed.

 

What does this support look like?

  • Using some of your funds for home and garden maintenance or assistance with dressing and showering
  • Practical help and information to manage at home, for example meal preparation or delivery, maintaining personal interests and hobbies 
  • Taking some time out to enjoy flexible day, evening, or overnight breaks to support you if you are caring for another person
  • Assistance to join a social group to help build your skills, and to meet new people

What steps do I need to take?

  • Usually your health professional may suggest an assessment from your local council’s Aged Care Assessment Team (ACAT) to determine if you are eligible, or you can call your local council to seek advice
  • If you are eligible, annecto will call you to set up the first meeting
  • At this meeting, annecto staff will ask you what you like: your history, your personality, and your dreams. From this conversation plans are made to develop a personalised and diverse set of supports. You determine what you want; what stays in, what goes and who should know about it.
  • Depending on what you want, annecto will connect you with other community links and providers to help provide ongoing assistance to ensure seamless support 

I want to manage my own support. What do I do?

Depending on the type of allocation of your funding from the Department of Health and Ageing, you have a choice to determine what you need so that you can do the things you want to do.

Using flexible funding arrangements means we can hold the funds for you while you advise how and what type of supports are required.  And when you feel ready, you can take responsibility for all or some of the funding, payments, staff rostering and staff employment while we provide mentoring and guidance.

There are times where you may feel you need further assistance in managing your own support. Should this happen; you should know that we can step back in and out as a flexible arrangement - as and when you need it. 

Mary used to go to the theatre often, but her children now live interstate and she didn’t feel safe going on her own at night. Catching public transport was also a concern as she recently started using a walking aid. annecto worked with Mary to find someone she got along with to pick her up and attend the theatre with her before dropping her home. It’s the companionship that Mary values the most.

annecto found a support worker with appropriate language skills to assist a group of Italian women attend their local church each Sunday.
